It should be come more clear as the story progresses (especially in chapter 3), but it's my bad if I haven't made it clear in the story itself.
In this world, gender doesn't work the same way it does in ours. In this world, people don't assume gender from appearance. So if someone was seen wearing a dress and pink lipstick, no one would assume that meant they were feminine/femme/a woman. In this world, the many genders that exist are recognized and not assumed until someone tells you what gender they are.
And the guard didn't ask Maira what her gender is because they're being very, very rude to her.
As for how Maira knew, that also should've been something I made more clear as well. The name/address he gives for deliveries gives her that information. My apologies if that didn't come across!
